I/We hereby agree to enroll the applicant as a student in the Winter Training Camp Program. I/We agree to be governed by the financial obligations and Academy regulations as stated below.

1. PAYMENT: Registration confirmation will be sent via email with a credit card authorization form with 50% down payment, the remaining payment is due three weeks prior to arrival.

2. Winter CAMP TUITION:  Tuition covers all golf program fees (including golf tuition, physical training and mental coaching) and room and board (for boarding students) at the Academy.  Expenses for extracurricular activities are NOT included, nor are incidental expenses, and are the sole responsibility of the applicant.  By signing this agreement, I/we agree to the full terms of this contract.

3. CANCELLATION POLICY:  In the case of cancellations made 30 calendar days or less prior to the scheduled date of arrival, no fees will be refunded. Cancellations made more than 30 days prior to the scheduled date of arrival result in a 75% refund. 

4. MEDICAL ATTENTION:  I/We hereby authorize representatives of the Academy to secure emergency medical care if necessary.  I have signed and submitted a Medical Waiver form to GGGA and accept responsibility for any and all medical expenses that may be incurred.

5. CONSENT & ACKNOWLEDGEMENT OF RISK & RESPONSIBILITY:
a) I freely and voluntarily assume any risk/hazards inherent in the program/activity and understand and acknowledge that my child may suffer personal and potentially serious injury due to an unforeseeable event associated with his/her participation.
b) My child has been informed that he/she is to abide by the rules and regulations of the Gary Gilchrist Golf Academy.  Upon arrival students will be informed of Academy rules and regulations.
c) In the event my child fails to abide by these rules and regulations, disciplinary action may result in his/her exclusion from further participation, or that the student may be sent home.  In the event that a child is sent home, that will NOT be accompanied by a chaperone and I accept full responsibility for any costs associated with sending my child home.  I agree to support the decisions of Academy staff in holding my child accountable for his/her actions.

6. PHOTO RELEASE AND WAIVER:  I/We hereby understand and agree to allow the Academy to use my child’s photograph in promotional material produced by the Academy including, but not limited to, the Academy’s Internet site, brochures, advertisements, and video.
